Central Catholic License Plate
Official State Central Catholic License plate order forms are in! Order yours officially by printing and completing the Official State Application Form! The price is $30.00 for each plate ordered. If you have already sent in money, you will not need to send anything additional. Once this form is completed, please return it to the Central Catholic Guidance Office or mail it to:

- Attn: Kevin Lynch
DO NOT MAIL THIS REGISTRATION FORM TO THE STATE. Return it to Central and we will submit it to the state. Please make all checks payable to ‘Central Catholic High School’. Your plate will be mailed to your address specified on the form. Nothing further will be required.
The fee for additional registration cards IS NOT included in the $30.00 price. If you need an additional card, please attach a brief note to your order, along with the additional $1.50 per registration card.
